Child Care Referral In-Take Form
Our mission is to provide families information about child care options that will meet each family’s unique needs. CCR&R can provide you with information that will help you make an informed choice about your child’s care.
Resource Technicians are available for individual consultation to provide families free customized referrals to licensed and legally operating child care facilities.
We have three sites in which referral services can be rendered:

Referral services are provided at no cost.
CCR&R only refers parents to legally operating programs, which include; legally operating half-day preschool programs, licensed child care center, licensed family child care homes, registered drop-in program and legally operating after-school programs. You may see the above types of child care arrangements on your referring listing. Licensed family child care homes and licensed child care centers are rated on a Star Rated License system. You may see a rating of a 1 all the way to a 5 on your referral listing. You may see a Temporary or a GS 110 license. Each referral mailed will include consumer education materials that are relevant to your family’s unique needs. Please let us know if you need more information about a particular resource.
Before you use our service, please understand CCR&R provides referrals, not recommendations for child care. The referral is based on the information received from the parent and from the provider. CCR&R does not license or endorse any child care program. All licensed and registered child care is monitored and regulated through the Division of Child Development. CCR&R cannot guarantee that a child care space will be available with the providers on your referral listing. The information that CCR&R uses to process a referral is kept confidential. Specific client information is not released to a third party and is kept secure in a pass coded database.
